Tax Transcript Request	If you do not have a copy of your tax return, you may order by calling 1-800-829-1040 or by completing a 4506-T (Tax Transcript Request form)	2008-2009

HOPE Scholar Evaluation	This form is to be completed by students who are enrolling in an Associate Degree program or have reached the designated checkpoints for the HOPE scholarship review. These checkpoints are 45, 90, and 135 attempted hours. Please complete this form and submit it to the Registrar’s office.

Georgia Residency – Student	This form is to be completed if requested by the Financial Aid Office to verify student residency
Georgia Residency – Parent	This form is to be completed if requested by the Financial Aid Office to verify parent residency

Satisfactory Academic Progress Appeal	This form is to be completed if you wish to appeal your Satisfactory Academic Progress status as defined by school policy

Federal Work-Study Application	This form is to be completed if you are interested in applying for a Federal Work-Study position. Student must be Pell eligible and enrolled in a Pell eligible program to participate.
Federal Work-Study Timesheet	Timesheet used by all approved Federal Work-Study positions

Local Scholarship Application	This application is to be completed by students who wish to apply for local scholarship funds to be awarded each quarter
Separation Status Information Request	This form is to be completed by student and/or parent (if Dependent) if they indicated their marital status as Separated on their financial aid application.
